Assisted Self-Care is defined as…

A person who is able to understand and act accordingly with most verbal communication by staff and other providers. He/She will be able to communicate their wants and needs to staff verbally majority of the time, Capable of walking and transferring independently (may include walker) He/She is capable of eating, dressing, grooming, bathing, toileting with supervision (does not require assistance) and/or has full control of bladder and bowel function.

 

Assisted Total-Care is defined as…

A person who is dependent on others for care and to oversee their personal safety. He/She will be capable of walking with assistance and/or using a wheelchair with or without assistance. They require assistance with 2 or more ADLs (eating, bathing, dressing, toileting, transferring and walking) May also require assistance and supplies to manage incontinence of bowel and/or Bladder. Requires occasional verbal redirection by staff for safety due to behaviors (Anxiety/wandering/sun downing)
