Memory Care Defined:
This person is dependent on others for care and to oversee their personal safety (up to and including exit doors that are alarmed and/or locked). He or she will require assistance with two or more ADL’s (eating, bathing, dressing, toileting, transferring and walking, continence). Requires staff assistance and supplies to manage incontinence of bladder or bowel or both. Requires frequent redirection and/or intervention (hands-on or limited 1:1 assistance) by staff due to ongoing confusion, disorientation, anxiety, agitation, refuses care, refuses medications, sundowning syndrome, exit seeking and other disruptive/repetitive behaviors.

*- Room & Board includes furnished room (includes bedding and linens), all utilities (including broadcast TV channels via facility antenna, cable TV is available for extra cost), standard meal service (3x daily plus snacks any time), standard medication management (up to 8 medications administered 2x daily, taken whole, swallowed with water or juice), weekly laundry service, daily access to bathing facilities and 24 hour supervision by staff.
**- Person Centered Pricing Plan will be based on a comprehensive care needs assessment that is unique to every individual. As such, each resident will only pay for the specific services that they need. Care needs assessments will be reevaluated twice annually or as needed based on changes in condition.
